Data Subject Application Form Within the Scope of Personal Data Protection Law

Personal Data Protection Law No. 6698 (Law) granted certain rights to the persons defined as “data subject” regarding the processing of personal data in the Article 11 of the Law. Pursuant to the Article 13/1 of the Law; the applications regarding these rights to be made to Codeway Dijital Hizmetler Anonim Şirketi (Company) who is the data controller, must be submitted in writing or by other methods determined by the Personal Data Protection Board (Board). Within this framework, the applications to be made to Company “in writing” must be submitted with a printout of this form, by personally or through a notary public or by signing with the "secure electronic signature" defined in the Turkish Electronic Signature Law No. 5070 by applicant and sending it to the Company's registered e-mail address. Below, specific information is given on the application by channels regarding how written applications shall be delivered to us.

The applications submitted to us shall be responded within thirty (30) days from the date the request is received by one of the methods specified above, in accordance with the Article 13/2 of the Law. Our responses will be delivered to the applicant in written form or in electronic form in accordance with the Article 13 of the Law. In case the request requires an additional cost, according to the Communiqué on the Principles and Procedures For The Request to the Data Controller published by the Board, 1 Turkish Lira may be charged per page over ten pages; and if the response to the application is given by means of recording medium such as CD or flash memory, the fee to be charged by data controller cannot exceed the cost of the recording medium.

This application form has been prepared in order to determine your relationship with our Company, if any, to determine your personal data processed by our Company in full, and to respond to your application accurately within the legal time period. Our Company reserves the right to request additional documents and information (copy of identity card or driver's license, etc.) for identification and authorization detection, to eliminate legal risks that may arise from unlawful and unfair data sharing and especially to ensure the security of your personal data. In the event that the information regarding your requests submitted within the scope of this form is not accurate and up-to-date or an unauthorized application is made, our Company does not accept any liability for claims arising from such false information or unauthorized application. Your personal data shared in this form will be processed solely for the purposes of evaluating, concluding your request under the Law, managing the entire process and communicating with you for these purposes. Depending on the nature of the request, your request may be transferred to our lawyers, our business partners and service providers providing services in the fields of complaint management, quality control, audit and risk analysis, legally authorized public institutions and private persons.

I acknowledge, declare and undertake that the application I made pursuant to the Personal Data Protection Law be concluded within the framework I have stated above, the information and documents I have stated in this form are accurate, up-to-date and belongs to me.
